Newcomer Olivia Rodrigo and K-pop band BTS were also among the big winners on Sunday at the first major awards shows in New York to be held with a full audience of fans and celebrities since the COVID-19 pandemic.
Bieber, 27, returned to the VMA stage at the Barclays Center in New York City for the first time in six years and took home Moon Person statuettes for artist of the year and best pop for his single Peaches.
"Music is such an amazing outlet to be able to reach people and to be able to bring us all together," said Bieber, referring to the dark days of the pandemic.
Last year the VMAs took place in New York without a live audience and with most performances recorded in advance because of the pandemic.
Lil Nas X, 22, a gay, black musician who had a breakout hit with Old Town Road two years ago, won video of the year for his gay rights anthem Montero (Call Me By Your Name).
"Let's go gay agenda!" said the musician, accepting the award.
Alicia Keys sang Empire State of Mind outdoors against the backdrop of the New York City skyline, while Doja Cat, Ed Sheeran, veteran New York rapper Busta Rhymes, Camila Cabello, Shawn Mendes and Machine Gun Kelly were among those on stage.
Sunday's ceremony also marked the 40th anniversary of the launch of MTV as a channel dedicated to playing music videos.
Madonna, also celebrating her 40 years in the music business, opened the show in a surprise appearance.
"They said we wouldn't last. But we're still here," Madonna said.
Jennifer Lopez and Cyndi Lauper sent fans wild with unexpected appearances, with Lauper weighing in on abortion rights saying that women want "control over our bodies".
Rodrigo, 18, took home three awards, including best new artist and song of the year for Drivers License in the fan-voted awards, capping a break-out year.
"This is so insane. This has been the most magical year of my life," Rodrigo said, dedicating her win to "all the other girls who write songs on their bedroom floor".
BTS was named group of the year and won best K-Pop for single Butter, and sent thanks by video from South Korea.
Rock band Foo Fighters got the Global Icon Award recognising their 25 years in the music industry.
The 12 time Grammy-winning band performed a medley of hits with "Charlie R.I.P." scrawled on their drum kit in tribute to the late Rolling Stones drummer Charlie Watts.

The MTV VMA's have always been one of the big red carpet events of the year to watch as artists really go all out, pushing fashion limits and creating styles.
In the last year, we've seen red carpet events cancelled and shifted to online versions which have not been nearly as exciting or controversial, so it's great to see the VMA's take centre stage once again.

Britney Spears has revealed she is engaged to partner Sam Asghari after more than four years together.
The pop superstar, who recently celebrated a legal victory in the conservatorship that controls her life and career, shared a video of her wearing a diamond ring.
She wrote on Instagram, "I can't f*****g believe it" alongside diamond ring emojis and a love heart.
The video featured an emotional-looking Spears, 39, showing off the ring while Iranian actor and fitness instructor Asghari, 27, asked if she is pleased with it.
"Yes!" Spears replied. She also planted a kiss on his cheek.
The couple met on the set of a music video in 2016. Spears recently praised Asghari for his support during her "hardest years".
Asghari's manager, Brandon Cohen, confirmed the engagement to People.
He said in a statement he is "proud to celebrate and confirm the engagement".
He added: "The couple made their long-standing relationship official today and are deeply touched by the support, dedication and love expressed to them."
The engagement news comes after Spears' father Jamie petitioned a court in Los Angeles to consider terminating her conservatorship.
The singer's personal affairs and money have been controlled by a complex legal arrangement since 2008 after she suffered a series of mental breakdowns.
Spears had been demanding the conservatorship be brought to an end and accused her father of being abusive in his role overseeing her estate.
At a hearing in June, Spears alleged it was preventing her from marrying Asghari and starting a family together.

Australia has made its decision: Bella Taylor Smith has been crowned The Voice 2021.
Along with the illustrious title, the incredible singing teacher takes home $100,000 prize money and a recording contract with EMI Recorded Music Australia.
After receiving the life-changing news from host Sonia Kruger, an emotional Bella said: “I really can’t believe it. I’m so thankful. I can’t wait to see what incredible things are ahead for me. I’m really grateful for you [coach Guy], for everyone who voted and for my beautiful family who I love.”
Coach Guy Sebastian was moved by his protégé’s achievement. “I’m so proud. There are so many deserving people up there, but you are special Bella, go out there and kill it, I know you will.”
Still in awe of her dream come true, the 23-year-old Sydneysider took to the stage to perform her new single Higher (stream/download here).
From the moment she stepped on stage for her first Blind Audition, Bella’s breathtaking voice captured the hearts of the nation, along with all-star coaches Rita Ora, Keith Urban, Guy Sebastian and Jessica Mauboy.
Last night, she wowed with her show-stopping final solo performance of Never Enough from The Greatest Showman.
Keith said: “If you’re not a believer at this point you’ve got to check your pulse. That was amazing.”
Rita was captivated and “felt like she was floating” calling the performance “unbelievable.”
Bella was joined by Guy on stage for an incredible duet of The Prayer by Andrea Bocelli, as the superstar coach surprised Bella, inviting her to join him on tour as his support act.
Bella said: “Guy’s been so lovely and genuine this whole process and I’m really grateful he’s been my mentor. I feel very lucky to be able to sing with him.”
